you didn't want to use the driver again, did you? If so, you could press the sleeve onto the shank, remove the screw, then see if you can pull it back off
Hold the driver vertically, rest the bottom flat edge on a table, whack the top of the driver with a hammer. Might even get away with simply slamming the flat side of the driver handle down on the table.
